I am aware of the risks connected with attending this event on this date. I agree to not hold accountable or bring legal action against the company, its staff, agents, or employees.

I certify that I voluntarily participate in this event and thus I acknowledge the risks of injuries involved in the event including, but now limited to muscle tears, sprains, and other physical injuries.

I am aware of the $5 payment for audition registration and $10 payment for the class workshop for 18+.

I will make every effort to obey the policies, and all safety rules, and will ask for clarification if needed.

By signing this agreement I agree to hold the entity entirely free from any liability, including financial responsibility for injuries incurred, regardless of the reasons or circumstances before, during, and after the event.
